Please could you inform me of your proposals for the future organisational structure of all current PCT provided community services? Please provide a description of your plans, including details of which of the options outlined in the document your PCT is.

Please provide me with, for 2009/10: 1. The number of patients who transferred from the Meads surgery to Bird In Eye surgery. 2. The number of patients who transferred from Buxted surgery to Bird In Eye surgery. 3. The number of patients who transferred from Bird In Eye surgery to the Meads surgery. 4. The number of patients who transferred from Bird in Eye surgery to Buxted surgery.

I would like to make a freedom of information request about prescribing advice issued by the PCT in relation to 'specials' (unlicensed medicines manufactured for a specific patient who has special requirements that cannot be met by use of a licensed medicine). Could you let me know:1. For the most recent period for which figures are available, how much did the PCT spend on specials? 2. How much does the PCT estimate can be saved by more effective prescribing in primary care? 3. What work has the PCT undertaking to reduce spending on specials by promoting more effective prescribing in primary care?

What has happen to the money from central government to every pct specifically to fund breaks for carers. I'm a carer and no information has been disclosed of this funding who controls it?

This is regarding the National Strategy for Carers. My understanding is that PCTs have had money from central government to fund breaks for Carers and I requested information as to the scheme that our PCT may be running.

Q1. The amount spent on recruitment consultancy services (not including clinical, community carers or nurses) for the financial year beginning April 2009 to present. Q2. If possible, a short description of the work carried out by each of these companies/individuals.

1] The total number of employment grievances lodged against the Trust during the financial year 2008-2009, not including accident claims.2] The total number of employment grievances that progressed to an Employment Tribunal during the financial year 2008-2009 and of these claims: i) the number of cases that concluded and were successfully defended by the Trust at Tribunal; ii) the number of cases that concluded and succeeded against the Trust at Tribunal; iii) the number of cases that concluded and which resulted in a settlement; iv) the total amount of settlement paid or agreed to be paid by the Trust for cases concluding during the financial year 2008-2009. 3] The total number of employment grievances that concluded and were compromised during the financial year 2008-2009. And of these claims: i) the total amount of settlement paid or agreed to be paid by the Trust. 4] The total sum of money paid or agreed to be paid by the Trust for legal advice and work relating to employment issues during the financial year 2008-2009. (This is typically overseen by the Trust's human resources or personnel department.) 5] The total number or proportion of employment disputes lodged against the Trust resulting in agreements containing a 'non-disparagement clause' during the financial year 2008-2009. (This should include all settlements achieved by way of COT3 and Compromise Agreements.) 6] Out of all employment disputes lodged against the Trust, the number of cases resulting in re-instatement or re-engagement during the financial year 2008-2009.

This request for information relates to the following years:- 2004-2005, 2005-2006, 2006-2007, 2007-2008 and 2008-2009. 1 What was the PCT budget for these years? 2. What was the yearly cost of administering the budget? Covering Accommodation, Utilities, Wages, and Stationary etc. 3 how many people were employed by the PCT in each of these years? 4 What was the yearly value of bonuses paid to directors?

Please provide me with a copy of your drug formulary or joint hospital formulary and a copy of the list of products available on your Pharmacy Minor Aliment Scheme if you provide this enhanced service.

With regards to my request, I’m sure that the council does not hold every single or exact job title but can you please find the person that holds and is responsible for that position. 1. Does the fertility department use (or has the department in the past used) pornographic material for patient use? 2. If yes then what forms of material are available (e.g videos, magazines)? 3. And from which suppliers? 3. How much is spent on pornography a year by the institute (statistics for each of the past 5 available years if possible)? And how much of this is public and private funding?

I would like to know how much the trust spent on design and implementing the changes (ie new signage around any buildings, on vehicles, equipment, staff name badges, stationery, the trust’s website etc). Could I have the amounts spent on rebranding in 2008/09 and in 2009/10. As we come to the end of 2009/10 I would be happy to receive the amounts that had been budgeted for to be spent on rebranding in 2009/10. I would also like to know any amounts budgeted to be spent on rebranding in the forthcoming financial year (2010/11).

The number of patients diagnosed with eczema, asthma and cancer in the years 2005, 2006, 2007, 2008 and 2009.Where possible, please provide this by email in an excel or word document.

What is your final decision for the future organisational form of your PCT- provided community services? For example community foundation Trust, Social enterprise, integration etc.  If you have chosen integration, please can you tell me the organisation which you have chosen to integrate with? When will this change take effect? Will it be before the April 2011 deadline?

a) I would like to know the names and details of those earning over £200,000 and an anonymous list of those earning between £100,000 and £200,000 b) For 2008/09 I appreciate that names and salaries of senior staff is already published in your annual accounts and that details for 2009/10 will be published later this year. However I would still like the information on pay to consultants and other non-board members for both these periods as that information is not included in your published accounts.

Under the Freedom Of information Act I am interested in receiving any guidelines you may have in regards to the prescribing of Glucosamine Sulphate or Glucosamine based products.

If a product is not referred to on the prescribing formulary list does that mean that it is not on any formulary (hospital or family planning)?

I am requesting:1. For each financial year 2007-08 and 2008-09, and as per the definition given above, the number of board meetings held by your NHS Trust. 2. For each financial year 2007-08 and 2008-09, and as per the definition given above, the number of board members invited to attend each NHS Trust board meeting. 3. Details of how board members are supplied with board papers for each board meeting. 4. An approximate figure for the average number of pages used in each set of board papers.

This is a freedom of information request regarding the availability of mental health services to older people which you kindly responded to. The original question asked whether these services catered for ‘mixed ages’; however, this was rather misleading and should have stated, as below, ‘mixed ages (i.e. under and over 65)’.
