1...Does the Trust hold and or receive and or keep copies of information detailing a GP's total annual NHS earnings. It is my understanding that this information is contained in a document known as an annual declaration of superannuable earnings.
2...Could you please state what are the current highest NHS earnings which have been declared by a GP on an annual declaration of superannuable earnings. Could you plesae provide the actual and most up to date figure? Can you please state what year these figures relate to. Could you please provide details of his or her superannuable earnings for each of the previous three years.
3...In the case of the above can you please state how many different practices the GP is connected to within your area. Can you please explain his or her link to each practice. For instance are they the senior practitioner. Do they own the practice? Can you please indicate what type of contracts the GP has with the primary care trust. These may be linked to actual practices or they could involve the provision of additional and or more specialised services. Can you please state for how long he or she has had these different contracts?
4...Can you please indicate whether the GP is linked to practices and or medical service providers outside the area. Can you please identify the number of practices he or she is linked with and state which part of the country they are in.
5...Can you please provide a description of what other work the GP has carried out on behalf of the NHS and which is detailed on the annual declaration of superannuable earnings. Was this work carried out within your trust area and or locally.
6...Can you please state how many individual GPs other than the one outlined above have declared superannuable earnings in excess of £200,000 a year. I am looking for your most up to date figure.

1. How many GP practices in the PCT achieved sufficient uptake of swine flu vaccination to qualify for easement of the PE7 and PE8 thresholds?
2. How many GP practices in the PCT are eligible for additional pay as a result? (Ie, how many practices achieved easement of the PE7 and PE8 thresholds and had patient experience scores which were below the original target but above the lowered target?)
