The patient population covered by the PCT; The number of pharmacies within the PCT; The number of GP surgeries and GPs within the PCT; The PCT’s total budget; The PCT’s total spend on primary care services commissioning, also broken down into the spend on pharmacy services and the spend on GP services (and other providers); The PCT’s total spend on practice based commissioning, again broken down into pharmacy services and GP services (and other providers); How many pharmacist sit on the PCT’s professional executive committee, and how many GPs?; How many pharmacists are involved in practice based commissioning decisions at the PCT and how many GPs?
What is the average time it takes for the PCT to reimburse pharmacies taking part in enhanced services?
I would also like to request some information on recorded deaths and illnesses in the PCT over the last two years (2008/09 and 2007/08):
Incidence of and deaths due to lung cancer; Incidence of and deaths due to all smoking related diseases; Incidence of and deaths due to diabetes; Incidence of and deaths due to clinical obesity; Incidence of and deaths due to cardiovascular disease.
I would also like to request specific information on the commissioning of four pharmacy services over the last two years (2008/09 and 2007/08).
I would like the following information on any smoking cessation, weight management/obesity management or equivalent, and vascular risk assessments (as introduced by the Government this year):
The total spend on the service (both the maximum allowed (budgeted?), and the proportion spent so far)
The amount spent on services commissioned for GPs and, separately, for pharmacies
The number of GPs and, separately, the number of pharmacies commissioned to provide the service
The fee paid to GPs and, separately, to pharmacies per treatment provided, or details on the fee amount and structure if it is not paid per treatment
The length of the contract for the service
The number of patients so far seen for the service in GP practices and pharmacy (or latest estimate)
And on any pharmacy minor ailments service:
The total spend on the service
The number of pharmacies commissioned to provide the service
The fee paid to pharmacies per treatment provided, or details on the fee amount and structure if it is not paid per treatment
The length of the contract for the service
The number of patients so far seen in pharmacy (or latest estimate)

 Whether the Trust has allocated resources to NLP training within the last three financial years (e.g. by sending staff on training courses).The specific dates of interest are between 31st April 2006 and 31st April 2009.I am interested in NLP training for staff development as well as for service provision. If so, what types of health care professionals were trained.   I am trying to ascertain the use of meditation techniques in the NHS across the UK.  I have placed a parliamentary question to the health minister who has informed me that this is a matter for local trusts.
I believe that meditation has a role to improve both mental and physical health.  I would like to know:
-         if meditation is used, encouraged, or promoted amongst your staff?
-         how many staff you have that are trained in meditation techniques?
-         how many patients have received meditative treatment?
-         is this number increasing or declining?
-         have you undertaken any research or assessment of the benefits of meditation?

 If I live in the areas served by East Sussex Downs and Weald PCT, and Hastings and Rother PCT, and am referred to an Ophthalmology clinic, where am I sent?  I am aware that patients Choice means that theoretically a GP could refer a patient to an Ophthalmology clinic anywhere in the U.K, but for the purposes of the Directory of Services that I am compiling, I need to know about the facilities that are available locally to visually impaired people living in the areas served by East Sussex Downs and Weald PCT, and Hastings and Rother PCT. Please can you provide the contact details - name of facility, and location (address with postcode) - of all Ophthalmology clinics (both in and out patient) in the areas served by East Sussex Downs and Weald PCT, and Hastings and Rother PCT.

 In the 08/09 financial year how much money did you pay to GP practices as a reward for a lowering in the prescribing of antibiotics drugs? Please provide details of the scheme and in particular, what drugs were covered by the scheme and  what reduction in prescribing rates was required to trigger 100% of the bonus scheme? Which GP practice received the most money from the PCT for lowering its antibiotic prescribing rates? Please give name and address of practice as well at the rate by which it reduced its prescribing (in absolute and percentage terms )and the cash reward it received from the PCT for doing this? Please give the same details but for the GP practice that reduced its prescribing rate by the most in percentage terms even if this was not the biggest in terms of absolute reduction. NOTE: If your PCT does not operate any such incentive scheme for the reduced prescribing of antibiotics then please answer ‘not applicable’ to all the questions.
