Where there has been correspondence between the PCT and East Sussex County Council which refers to the situation endured by pedestrians during the recent freezing weather I would like a copy. Likewise, I would also like to see any notes created as a result of any meetings or telephone calls at which representatives of the PCT and East Sussex County Council were present to discuss the consequences of the freezing weather.

To Whom It May Concern, I would like to make a request for information. I would like to know how many retired General Practitioners receive an annual pension of £33,000 or higher, net of lump sum. I would also like to know the highest pension paid to any General Practitioner in pensions, net of lump sum.

Could you please supply me with the current guidance, recommendations or protocols that you provide to prescribers, with respect to the use of melatinin or Circadin 2mg tablets in licensed and unlicensed indications. Please include copies of material which you hold in the form of paper and electronic records including emails.

1.  What assessment has the PCT made about how general practices’ payments for 2010/11 will be affected by the changes in prevalence formula (in comparison to 2008/9, 2009/10)? How has the PCT communicated information about changes in payments to practices? 2.  If the PCT will be paying out a lower amount overall because of the change, how will this funding be reinvested in primary care? 3.  What measures has the PCT agreed to offer additional support to practices that experience cuts to funding (compared to 2008/9, 2009/10) under the prevalence changes? If so, how many practices will benefit from this? 4.  If the PCT will be paying out more overall to practices after the changes to the prevalence formula take effect, how will this be funded? What sort of agreement does Uckfield Hospital have with Cooper & son Funeral Directors also of Uckfield?

Please can you answer the following questions on GP balanced scorecards under the Freedom of Information Act. 1. Please can the trust provide a copy of the most recently published balanced scorecard for assessing GP practice performance. (Please note: If your trust has yet to publish a scorecard, please could you advise on when you expect to). 2. Has the trust modified any GP contracts as a result of issues which have arisen from measuring performance using balanced scorecards? If so, please provide details. 3. Please note that I am not seeking the names of any individual GP or doctor within your area.  Ideally I would like up to date figures as regards earnings. But if these are unavailable could you please provide the most up to date figures you have.1....Could you please state the highest annual income/salary currently being received by a family doctor and or GP in your area.  Could you please state how many individuals are in receipt of this level of payment?  Can you please state for how long each of these GPs has been in receipt of an income at this level?  By income I mean the actual earnings of a particular doctor. This will not include payments made to him or her to help with actual surgery/staff costs. Rather this will be their actual income and salary and will also include any additional 'over-time' and weekend payments as well as payments made in relation to the completion of particular targets.  It will also include payments made in relation to the treatment of particular conditions. 2....Is this the highest annual salary ever paid to a GP in your area?  If not can you please state what is?  Can you please state when this salary was paid?  Can you please state how many GPs were in receipt of a salary at this level and for how long. 3....Could you please state how many family doctors/GPs in your area are currently earning £190,000 a year or more.

I would also like to add a further request, for information regarding Uckfield Hospital. What would happen to a body, if someone died at the hospital during office hours? Would the same procedure be put into place, with Cooper & Son collecting the body or, would some other procedure be put into place, and if so, what?

Premature Birth Survival - Have any babies survived at your trust in the last three years after being born following less than 24 weeks gestation in the womb? If the answer is yes please provide the following. How long in weeks and days did the baby spend in the womb? What sex was it? When was it born? How much did it weigh?

Pleased advise the work rooms in the building none as St Mary's suite EDG Hospital, these rooms are sub let to Social services department. There are 4 in total. The number of people who  could work in each room  in relation to law and regulations, the size of rooms. also the identified use of room ie office space/ kitchen? , which have unit 3 lighting. i would like information on the two rooms which come off the  2nd largest room. one of which i understand to be a walking in cupboard. Please advise of  opening windows , ventilation shafts etc whose responsibility is it to meet established guidelines if rooms are used by social services.
