 Musculoskeletal service survey.

 Please provide the following information: The number of girls given the human papilloma virus vaccine so far. The number of people offered the jab, and the number of refusals of parental permission you have had? The roll-out plan for the vaccine. The number of hospitalisations due to side-effects from the jab so far and the projected number at the outset.

 I would like to make a request for information regarding Private Providers from whom your organisation is already contracting or planning to contract services from.  I am currently updating our database and hope you will be able to provide me with the following:

 I would like to know how much the trust spends on telephone calls from the fixed-line phones to mobile phones.  Who currently provides the department's fixed-line service? How many minutes of fixed-line to mobile calls are used?

 1. I would like to know the total annual income, including bonuses, other awards, and overtime for the highest paid GP in your trust. 2.  I am currently conducting research in order to better map the weight loss management programs across the nation. Could you let me know whether, as a PCT, you advocate a particular weight loss management scheme for physicians to refer patients to? I am looking into programs that have direct measuring and tracking of each member.

 I would like to request a list of any schemes offered by the trust under which patients are offered incentives for healthy behaviour.Healthy behaviour would include, but is not limited to, quitting smoking, losing weight and eating or drinking more healthily.Incentives would include cash, vouchers, items or any other tangible benefit offered by the trust to induce people to behave more healthily.For each scheme run by the trust, I would like to request what the incentive is and what form of healthy behaviour it is offered for.
