 Please can you provide me with information on the services you provide for adults, young people and children who are blind or partially sighted. Please include services that are provided jointly with other agencies.

 1/ Please could you tell me how much the PCT has spent on external consultants and legal and professional fees since 1 April 2006. Please could you break down the response by financial year (ie Apr 06-Mar 07, Apr 07-Mar 08 and Apr 08 – Mar 09), and for each expenditure detail how much was spent, who was hired and summarise what the money was spent on.2/ With specific regard to the Equitable Access in Primary Care procurement for GP-led health centres and practices in under doctored areas, please can you tell me:a/ In what way did the PCT conduct the procurement for the Equitable Access in Primary Care programme – ie was this done internally, through a commissioning hub or with external support, and if so who?b/ How much has the PCT spent on the procurement/tendering process so far?c) How much more does the PCT anticipate spending on the procurement/tendering process?

 How many applications were heard by the Individual Patients Funding
Panel/Fundings and Appeal Panel between April 2008 and March 2009.
How many of those applications were refused?
How much money did the panel approve to spend on "exception" cases over
that time?

 Could I please request an electronic copy each of Human Resources Strategy, Training & Development Strategy as well as copies of the HR policies and any information on the organisation/structure of the HR Team's that you have?

 How many girls under the age of 21 have had cosmetic surgery on the HNS in the past three years? How many boys under the age of 21 have had cosmetic surgery on the HNS in the past three years? Please include: - separate figures for each year.- which hospital each of the operations were carried out.- the age of each of the patients of who have undergone surgery.- as much details as possible of the type of surgery each patient has undergone, eg, breast enlargement, breast reduction, ears pinned back, nose re-shaping.- if possible, the reason for the operation.

 We are unable to find a UK-wide list of the Pain Management Services or Clinics run in the NHS so we are trying to compile this information by contacting each individual trust.If you can provide this information, it would be appreciated, otherwise please forward within your trust to the relevant person or department.

 I am unable to find a national list of District Nurse Locality Managers so I am trying to compile this information by contacting each individual trust.

 We are aware in some NHS trusts they have Diabetes Specialist Nurse Educators, specialist diabetes nurses that oversee the training of diabetes nurses.
Do you have such people in your trust?
We are seeking to compile a national list of these people in order to ensure we limit our contact to appropriate staff.

 I would like to ask a series of questions relating to GPs' out-of-hours service and wages.
1. Firstly, I would like to know the highest amount a GP was paid for an evening or overnight shift in 2008 and the exact number of hours this shift comprised.
2. I would also like to know the highest amount  a GP was paid for a weekend shift in 2008 and the exact number of hours this comprised.
3. I would also like to know the highest amount a GP was paid for any shift undertaken during festive or holiday period in 2008 and the exact number of hours this comprised.

 Please can you tell me what Practice Manager Networks/Forums are currently in operation throughout your PCT and provide me with details i.e Practice name, Chair, members, contact details etc. The reason we are asking for this information is in an attempt to unite and link all existing smaller PMgr Networks to this national group to share best practice and ensure sustainability for all.
