5. In these years, how many complaints did you receive about nursing homes and care homes for the elderly from patients or their relatives, and how many were investigated?
6. In these years, how many complaints did you receive from GPs about nursing homes and care homes for the elderly, and how many were investigated?

2. Please provide details of the five most highly paid orthodontists (although I understand you may not name them), including the following information:
a) Their NHS earnings for the last three financial years
b) a breakdown of these earnings (i.e. how many practices/hospitals have they worked for? Which practices/hospitals?)
c) How long each has worked as an orthodontist for the NHS (if known)
d) Their job titles

161-10/11  a) Please confirm or deny wheather the PCT has a local policy that restricts access to long acting reversible contraceptive (LARC) methods. Please supply the PCT's policy on access to LARC methods.
b) Please supply the date and details of the most recent audit that the PCT undertook of the provision and accessibility of LARC.
c) Please confirm or deny whether the PCT has a formal strategy in place to reduce the number of repeat abortions. Please supply the PCT's strategy to reduce the number of repeat abortions.
d) Please supply figures for (a) the PCT's total spending on Community Contraceptive Services (CCS) and (b) its average spending on CCS per head of PCT female population aged 15-49, for the latest available period.
e) Please supply the date and details of the PCT's most recent local needs assessment in relation to all contraceptive service provision.
f) Please provide figures for the percentage of repeat abortions in (a) under 20 year olds (b) over 20 year olds in 2009.
g) Please supply figures for the percentage of all General Practitioner contraception prescriptions that are long acting reversible methods.
h) Please supply figures for prescriptions for each contraceptive method as a percentage of all General Practitioner contraception prescriptions.
i) Please confirm or deny whether the PCT has conducted an assessment of the number of healthcare professionals in the PCT who are accredited to fit all LARC methods. Please supply the findings of the PCT's assessment of the number of healthcare professionals in the PCT who are accredited to fit all LARC methods.
j) Please confirm or deny whether the PCT has conducted an assessment of the local need for specialist contraceptive training and weather the PCT has plans in place to increase capacity for specialist contraceptive training provision.

 174-10/11  I would like to know how many people in the last three years you have treated, in hospitals run by your Trust, who weigh 25 stone or more.
For each instance I would like to know the following information: -
1) The largest recorded weight of the patient;
2) The length of time he/she was treated in the hospital, in days;
3) Whether or not he/she continues to be treated in the hospital;
4) How much it has cost the Trust to treat the patient;
5) What illness or illnesses they were treated for while at the hospital;
6) Did any special measures have to be taken in order to accommodate the patient in the hospital and, if so, what they were;
7) The name of the hospital where they were treated.
NB: In your response, please list the information as follows:

 178-10/11  I wish to know the total expenditure your organisation has spent on printed materials in 2007, 2008, 2009.
By printed materials I mean items such as printed stationery, brochures, forms, leaflets, marketing materials, training materials, folders, posters, display and exhibition materials, banners, stickers, labels, etc.
I do not want details of any in house photo copying, print from an in house repro graphics unit, printed copies from desktop printers or networked multi functional devices, nor do I want spend on signage.
