Integrated Plan for Health, Social Care and Wellbeing in East Sussex

What is the plan?

East Sussex Primary Care Trusts (PCTs) and the council’s Adult Social Care, Children’s Services and Governance and Community Services Department have pledged to work together on a range of priorities.

This plan covers the next five years and describes the policies that govern and support our thinking and planning – and through which our performance will be measured.

What is the vision for this plan?

Why do we have it?

Developing closer working links between the council and health organisations in East Sussex can bring benefits to us all. 

The Plan identifies absolute “must do’s” for achieving closer integration in key areas. It does not replace our existing plans, which have been produced as a result of extensive consultation and engagement with local people, but will help take them forward. It will make sure that by working together we improve the quality of life for people in East Sussex.

This plan is underpinned by a strong commitment to succeed.
It will enable us to give greater emphasis to areas where joint working can have greatest impact on the quality of life for people in East Sussex, to drive up efficiency and improve support and service quality.

How will we know if it is working?

We will be judged by the Care Quality Commission (CQC) - the national organisation which oversees health and social care – on how well we work together with our partners, and on what these partnerships achieve.
